A list of your repeat prescriptions is printed on the white half of your prescription. Please use this tear-off sheet to request further prescriptions.

The date of your next medication review will also be printed on this sheet. If this date has passed you will be required to either see or speak to a doctor before more medication can be issued.

Please allow two working days for prescription requests to be completed. If you wish your prescription to be posted please provide a stamped addressed envelope.

You can request your repeat prescription in the following ways:

  • Request that a local pharmacy collects your prescription directly from us or nominate a pharmacy for your prescription to be sent electronically via EPS
  • Order it online through Patient Access (please note that you must be registered for online access. Please click here for further information)
  • Leave your written request at reception
  • Email your request to


Please note that unless you are housebound we will not be able to issue repeat prescriptions over the telephone.



Patient information: It is usual for the consultant to issue the first prescription.  You should, however, be aware that the Practice might not issue prescriptions that are “off-formulary” i.e. not standard NHS issue for the condition/treatment.

Medication Reviews
Please make a routine appointment if your medication review is due. We have to do this at least once a year in order to ensure that we are treating you appropriately.
